Welcome to the Bupa IB Onboarding Portal
This site has been set up to help Bupa IB practices submit the information we need to build your practice website within the Bupa IB-supported framework. There are two forms to complete:
1. Practice Details Submission Form
Use this form to provide information about your practice, including contact details, services offered, insurance and provider information, data tracking requirements, logos, brand colours and account details.
2. Staff Submission Form
Use this form to provide information about each staff member at your practice. Please complete a separate submission for each individual staff member.
Once we’ve received your submissions, we’ll use this information to build out your website. If you have any questions along the way, please get in touch with your PDM.
